JOB DESCRIPTION
Title: Occupational Therapist (O.T.)
Reports To: Program Director
Responsibility: Provides occupational therapy services
Specific Duties:
1. Responsible to review and initial all evaluations and discharge summary. The O.Т. will perform assessment and reassessment of all Participants' physical condition and functioning status. Responsible for the treatment of Participants to achieve and maintain maximum performance.
2. Responsible for the development of a treatment program when indicated, including long goals and providing treatment.
3. Responsible reaction for recording and signing treatments each day, noting method of treatment, of participant and narrative notes as needed; quarterly progress notes.
4. Responsible for developing personnel policies that define the Occupational Therapу responsibilities and the duties assigned to the occupational therapy assistant.
5. Responsible for the instruction, education and training of other Health Care team Professionals; a majority of whom are representatives of the center's multi-disciplinary and family members in methods of maintaining and improving the Participant's functioning level.
6. Responsible for the design and supervision of a maintenance program. In the absence trained the O.T. the Registered Nurse [RN] shall be trained by the O.T. to supervise staff to provide maintenance program services and carry out specific activities sheets developed and by the Occupational Therapist [i.e., group exercises; Participant monthly flow functioning. assist O.T. as needed] to help the Participant maintain current levels of functioning.
7. May be assigned other duties as assigned by the Program Director.
Occupational Therapist Qualifications:
1. Must Therapy be a certified Occupational Therapist registered with the American Occupational Association.
2. Should possess current CPR and First Aid certification [or within six months of hire]; in possess to current TB clearance [within the year prior to hire or within seven days of hire]. Must indicating individual health is exam [within the year prior to hire or within seven days of hire] capable of performing the job duties.
3. Must be a Team member and a professional is a member of the Multi-disciplinary Team meet [MDT] and participates as needed. The number of hours provided the PT shall meet the needs of the participants; at a minimum, and review and sign all evaluations and the discharge summary as required by Sections 54129, 54315 and 78067 and Title 22, CCR.
4. Able to bend, lift, and stoop effectively so as to adequately perform job duties and responsibilities.
